DUE TO INSURANCE REGULATIONS
Minor Release and Waiver of Liability and Indemnity Agreement Form needs to be COMPLETELY filled out by the parents of anyone
UNDER THE AGE OF 18 that wishes to enter the PIT AREA at Houston Motorsports Park. Both parents must sign this form and it must be notarized.
If one parent has sole custody then only the parent with sole custody has to sign.
This waiver is valid for the 2005 Calendar Year.
Please bring this Waiver of Liability to HMP and turn it in at the PIT OFFICE the next time you return.
HMP does have a NOTARY PUBLIC on staff, and as long as BOTH PARENTS are present (SARAH BAKER) can notarize the MINOR RELEASE form for you on the day of the event.
